If you would like to schedule an appointment, please call the office and speak with our staff. If you cannot reach the office due to it being outside office hours or phone system being busy, please leave a voicemail and your call will be returned as soon as available. If you are interested in telemedicine, please let us know when scheduling your appointment so that we may process your paperwork accordingly.
Note: Prior Authorizations are often required by the insurance companies to obtain certain medications. It remains the responsibility of the patient to ensure they have enough medication to last through their next appointment. Please address refill requests at least 10 days before the prescription expires so that the office has enough time to process the request. It is most efficient to call and have your pharmacy fax us the refill request.
